It has been estimated that out of 27 million pregnancies in India nearly, 189,000 occur in HIV + mothers leading to an estimated cohort of 56,700 infected babies (Joint Technical Mission on PPTCT). PPTCT program using Nevirapine was initiated in the country in 2001. However, by 2004, only 3.94% of all pregnant women received HIV counselling and testing and only 2.4% of the HIV-positive pregnant women received the antiretroviral drug prophylaxis. Currently, a total of 1880 PPTCT centres are functioning in the country which includes 502 stand alone PPTCT centres as well as 1380 ICTCs which offer PPTCT services. While the majority of these centres are in the public sector a few are also in the private sector.
It is estimated that to provide universal access to PPTCT services throughout the country, an additional 2140 centers will need to be put into place, many of which will be at the CHC level as part of the ICTC centres established there. In NACP III there will also be scale up the PPTCT services through public private partnerships. With the expansion, NACP3 aims to prevent vertical transmission of HIV in an annual cohort of 189,000 HIV positive pregnant mothers throughout the country.
A critical component of the service expansion will include strengthening the system of referrals and linkages to ensure completion of the full cascade of services: from testing pregnant women, providing prophylaxis to pregnant women diagnosed with HIV, advising about breast feeding, confirming the child’s HIV status, and enrolling women in appropriate ongoing care and treatment services depending on her stage of disease. Another challenge to extending services lies in increasing the proportion of women who seek antenatal care. |
